By checking the box below, I agree to the terms and conditions set forth by Buddha Pooch Dog Rescue Project: I agree to Buddha Pooch Dog Rescue Project’s adoption process and will undergo a home check and interview at the discretion of the organization. I understand that Buddha Pooch Dog Rescue Project will check my references, including veterinary and personal references, as part of this application. I acknowledge there is a NON-REFUNDABLE adoption donation of $450.00 associated with adopting a dog from Buddha Pooch Dog Rescue Project, which is tax-deductible under IRS 501(c)(3) guidelines. For tax purposes, please reference EIN 86-2878848. I understand that this adoption donation will ensure the organization can rescue another homeless dog in need. I agree to notify BPDRP ONLY by email (buddhapooch@gmail.com) and provide a 14-day timeframe for BPDRP to regain ownership of the adopted dog if I can no longer care for it. I understand that I may ONLY surrender the dog to BPDRP. I agree to indemnify and hold harmless Buddha Pooch Dog Rescue Project against any losses, lawsuits, claims, injuries, or damages incurred by me or to any persons or property by my adopted dog after the adoption is completed. I understand that Buddha Pooch Dog Rescue Project will disclose any known health or behavior issues regarding the dog before the adoption is finalized. I acknowledge that some of my adopted dog’s background, including health history, may be unknown, and I will not hold BPDRP responsible for any behavioral issues or medical expenses incurred after adoption. I understand that submitting this application does not guarantee approval for adoption.
